Forms are created in collaboration with providers. The DRO team ensures forms have consistent design and content, improving usability. Improvements are managed by the DRO team, meaning forms are kept up to date. Forms are available for anyone in BC to use. eReferral forms are created by specialty or program. Available forms can be viewed below or found in the Ocean Forms Library by searching “PHSA.”
eReferral forms are developed with input from family practice and specialists and can be used by specialists across BC.
Standard Referral - a general purpose referral form that can be used by any specialty or service area to receive referrals for patients.

Lab forms
The Standard Outpatient Lab Requestion (SOPLR) is in development with Provincial Lab Medicine Services and will be available soon.
